Transition Support Group (Preparing for Adulthood) Letter - Dates for 2022/2023
The Transition Support Group aims to provide parents and young people (aged 16-18) with the information they need, to help prepare them for transition from children’s social care to adult social care.
The Transition Support Group will be holding 7 meetings that focuses on topics selected by parents and carers who have accessed this project in the past. The group aims to provide a space for young people, parents and carers to discuss all matters regarding transition.
These meetings will be held online via TEAMS from 4:00-5:30pm and are free to attend.
We would recommend that all sessions be attended.
Please read this disclaimer carefully before you book a space:
- This is not relevant to you or your young person if they do not receive support from social care children’s services from the London Borough of Redbridge.
- Your young person may be receiving support from social care Children’s services; however, they may still not be eligible for Adult Social Care. This is subject to an assessment by the Adult Social Care team. If you are unsure whether you qualify, the Care Outcomes meeting on January 12th, 2023, will discuss these assessments.
- Your right to access Adult Social Care provision will not be affected if you do not attend these sessions.
