Transition Brokerage Project (Outward Housing)

What is this service?
To help Young People to identify the changes they want to make to their lives; find support services and community opportunities that the young people requires; negotiate with service providers and prepare community resources as necessary; cost and write a Person Centred Plan; and initiate implementation of the Person Centred Plan (once agreed by the Authority).
Who is it for?
Young people with disabilities aged 16 to 25
Outcomes for service users
Young People to have Person Centred Plan and appropriate provision in place for when they complete school / college.
Young People to be able to make informed choices and have their views heard and acted upon.
Young People to be able to explore the options available to them and make better informed decisions; and actively engage with decisions that are being made.
