District Nursing | Redbridge Community Health and Social Care Services (HASS)

The district nursing service, part of the Redbridge Community Health and Social Care Services (HASS), provides 24 hour care for housebound people aged 16 and over who have an identified nursing need and who live in Redbridge.

The service is provided in the patient’s own home, or in residential care settings, with the exception of leg ulcer clinics which are held in a health centre.

District nurses care for patients with a wide range of nursing needs including chronic disease management, palliative/end of life care. The service also provide nursing services to patients who are acutely ill and need intensive and technological care, or who have specialist care needs.

District nurses play a pivitol role as patient assessors and care co-ordinators as well as undertaking the role of educator, counsellor and advocate for the patient. District nurses will promote self care where appropriate to do so. The evening/night nursing service provides continuity of care for district nursing outside of normal working hours.
