The Child Death Review process

All deaths of babies or children aged under 18, in England, are investigated according to set procedures. A leaflet, written by The Lullaby Turst (formerly known as FSID), is available for parents and carers of a child under 18 who has died. It tells you what happens in the Child Death Review.

The death of any child is a tragedy. It is vital that all child deaths are carefully reviewed. This is so that we may learn as much as possible from them, to try to prevent future deaths, and to support families.

The leaflet describes stages of the investigations including the post mortem, professional meetings, inquests, Child Death Overview Panels and Local Safeguarding Children Boards. It also lists the contact details of organisations that can offer bereavement support.
