Applying for Care Packages/Short Breaks

Most short break services will accept direct referrals from families or a professional for a child or young person. There are, however, some services which can only be accessed through the Childrens Resource Panel.

If a panel request is required please follow the process below:

  • Make a referral to the CPAT (MASH) team.
  • The child or young person’s needs will be assessed by a worker from the Disabled Children's Service
  • The worker will make a request to the Resource Panel based on their assessment
  • The panel will consider the request and if appropriate an individual care package would be agreed. This would take into account the child’s needs, service availability and best value

Please note that there will be a waiting list for some of the Care Packages/Short Breaks.

Also your child will be subject to a Child in Need Plan, under the Children Act 1989 and this plan will be reveiewed on annual basis.
